The Well Society is required by law to maintain the privacy and security of your protected health information (PHI). We must:

• Follow the terms of this Notice

• Notify you if a breach of your unsecured health information occurs

We may use and share your health information for the following purposes:

1. Treatment

We may use your information to provide chiropractic services, KT taping, and related wellness services. We may share your information with other healthcare providers if needed for your care.

2. Payment

If applicable, we may use your information to bill for services or respond to inquiries related to payment.

3. Healthcare Operations

We may use your information to operate our business, improve our services, and ensure quality of care.

4. Legal or Safety Reasons

We may disclose your information when required by law, to prevent a serious threat to health or safety, or to comply with public health and reporting requirements.

Your Rights

You have the right to:

• Get a copy of your health records

• Request corrections to your health records

• Request confidential communications

• Ask us to limit what we use or share (we are not required to agree in all cases)

• Get a list of disclosures we have made

• Get a paper or electronic copy of this Notice

• Choose someone to act for you (medical power of attorney or legal guardian)

• File a complaint if you feel your privacy rights have been violated

You will not be retaliated against for filing a complaint.

We are required by law to:

• Maintain the privacy of your health information

• Provide you with this Notice

• Follow the privacy practices described in this Notice

Changes to This Notice

We reserve the right to change this Notice at any time. Any changes will apply to all health information we maintain. The updated Notice will be available upon request.

This Notice of Privacy Practices applies to all services provided by The Well Society, including community events, pop-ups, and wellness experiences.
